5th Annual National Healthcare Decisions Day - April 16, 2012

Do you know what health care treatments you would or would not want if you could not speak for yourself?  An Advance Health Care Directive allows individuals to make their end-of-life wishes known in the event they are unable to communicate. 

April 16, 2012 is the 5th annual NHDD.  NHDD was founded as a result of a growing concern that individuals over the age of 18 years old were not executing the proper legal documents so that their wishes are known.  NHDD was founded as a collaborative effort of national, state and community organizations committed to ensure that all adults with decision-making capacity in the United States have the information and opportunity to communicate and document their healthcare decisions.
